An individual falls assessment focuses on one person - their function, mobility, environment and specific falls risk. 

Our whole-home Falls & Function review takes a wider view. We explore patterns across a care service, considering falls data, changes in residents' function, everyday routines, staff practice, equipment, the environment and opportunities for movement. 

If you are seeing repeated falls or similar problems occurring across your service, a whole home review may be more appropriate. 

Falls risk assessments are an important part of care, but they do not always explain why falls continue to happen. 

We look beyond the individual risk scores and incidents to explore the relationship between the person, their function, mobility, everyday occupations, environment and the care systems around them. 

Our aim is to help you identify patterns and practical opportunities for improvement, rather than simply add another piece of paperwork. 

No. The whole-home falls & function review is a service level clinical improvement review and is not intended to replace individual assessment.

We may explore selected anonymised falls, functional themes and examples of care practice to understand wider patterns across the service. 

If we identify that an individual resident may benefit from a comprehensive Occupational Therapy or Physiotherapy assessment, we will discuss this with you. 

Yes. We provide practical, clinically led training for care teams. 

Our training includes falls awareness and prevention, function and mobility and meaningful activity. We can also develop targeted staff learning in response to themes identified through a clinical or whole-service review. 

We focus on helping staff to understand why something matters and how to apply that knowledge within everyday care. 

Availability will depend on the type of support required and our current clinical commitments. 

When you contact us we will discuss the level of concern and the timescale that you are working with. 

Our service is not an emergency or urgent response service. Immediate health or safety concerns should be managed via the appropriate NHS or emergency pathway. 

We can adapt training to the experience, role and learning needs of your team.

Before delivering training, we will discuss who will be attending and what you want staff to understand or do differently afterwards. 

This helps us ensure that the training is relevant to the realities of your service rather than delivering a generic presentation. 

Yes, but we won't recommend a bespoke package simply for the sake of it. 

Tell us about the problem that you are trying to solve and we will discuss the most appropriate level of support. 

This may involve a single assessment or training session, whole-home falls and function review or ongoing clinical improvement support. 

We are based in Gloucestershire and primarily provide face-to-face services across the county and surrounding areas. 

Some training, consultancy and follow-up support may be provided where appropriate.

Yes.

Our role is not necessarily to duplicate existing services. Where appropriate and with the relevant consent and information-sharing arrangements, we can work alongside existing health and care professionals. 

We aim to identify the most appropriate next step and will recommend referral or liaison with other services where this is clinically indicated.
